Are you eligible?

To find out if you are eligible to move into an established Perth Disability Home, please fill out the following questions and our support team will be in touch.

Eligibility is based on the following criteria

  1. A person/friends/family living with a complex disability and eligible for ‘high physical support’ SDA or ‘fully accessible’ SDA

  2. A desire to live as independently as possible

  3. Aged between 18-64 years

  4. A person/friends/family living with a significant disability, who isn’t an NDIS participant but supported through alternative means and desiring this type of housing.

Do you require 1:1 support for more than 5 hours per day and also need 24/7 back-up on call support?
Do you have a goal to live more independently in an apartment including using assistive technology and a shared model of support?
What approvals do you have in your plan?
